The worker suffered serious injuries including multiple spinal fractures and internal trauma.
The Department of Mines, Industry Regulation and Safety (DEMIRS) investigated the incident and prosecuted Northern Star Resources Limited under the Mines Safety and Inspection Act 1994 (WA) for failing to provide and maintain a safe working environment.
Northern Star expressed deep regret for the worker's injuries, accepted responsibility through its early guilty plea, and enacted comprehensive mobile equipment collision-avoidance and underground pedestrian exclusion zone protocols across all its Australian sites.
Carosue Dam Mining Complex Contractor Fatality Proceedings
In March 2026, WorkSafe Western Australia instituted formal regulatory charges under the Work Health and Safety Act 2020 (WA) against Northern Star Resources Ltd and an associated contract mining services provider in connection with a fatal incident involving a specialist contractor at the Carosue Dam gold operations.
These charges are currently unresolved. The commencement of a prosecution represents an assertion of statutory breach by the regulator; it does not constitute a finding of guilt, liability, or negligence against Northern Star Resources. The matter is currently scheduled for initial case management hearings.

Bronzewing Underground Operation (Historical Predecessor)
Media commentaries occasionally reference historical underground incidents in the Yandal belt, including a 2014 fatality at the Bronzewing site.
